Health Connection

Anyone can receive services at Health Connection regardless of age, sex, marital status, religion, race, ethnicity, color, creed, handicap, childbearing history, or income. Health Connection provides access to comprehensive sexual and reproductive health services (family planning). Services are provided on a sliding fee scale based on family household income and size. Services are provided on a voluntary basis in a confidential manner. Individuals who meet certain criteria will not have a charge for the services rendered. Fees are based on the federal poverty guidelines. Health Connection staff provides a good faith estimate based on the best information known and provided at the time of the estimate, it will not include any unknown or unexpected costs that may be added during your visit or treatment. The scope of services may change based on the patient's needs at the time of service, which will inadvertently affect the amount owed. You may ask for a good faith estimate by calling the clinic nearest you or in person at the clinic nearest you.

Basic Infertility

Include an initial infertility interview, client history, physical examination, limited laboratory testing, education and counseling, and referrals to a specialist as needed.
Medical Problem

A problem is defined as anything that the client and/or health provider perceives to be a health threat (such as abnormal discharge, urinary tract infections, breast lump, etc.)
Preconception

Preconception healthcare is intended to promote the health of individuals of reproductive health age before conception, with the goal of improving pregnancy-related outcomes.
Pregnancy Testing and Counseling

Counseling: Non-directive counseling on all options
Preventive

Detect health issues early on. May include a comprehensive health history, height, weight, blood pressure, health assessment, basic screening tests, immunizations, laboratory testing, and education and counseling.
Sexually Transmitted Infection (STI)

PrEP is a medication that users take daily to lower their chances of getting HIV.

Sexual Assault Services

Trained staff provide free, confidential services and support to anyone affected by sexual violence, inclusive of survivors of any age, race, gender identity or sexual orientation.

Funds are provided by Indiana Family Health Council, Inc., from the U.S. DHHS, PHS Title X Family Planning Project, and IDOH/MCH Family Planning grants in addition to patient insurance payments, fees, and donations. As public funding becomes more difficult to obtain, it is necessary for us to encourage clients to donate the maximum amount they can toward our services in order to insure the continuation of the program.
