Health Connection

Anyone can receive services at Health Connection regardless of age, sex, marital status, religion, race, ethnicity, color, creed, handicap, childbearing history, or income. Health Connection provides access to comprehensive sexual and reproductive health services (family planning). Services are provided on a sliding fee scale based on family household income and size. Services are provided on a voluntary basis in a confidential manner. Individuals who meet certain criteria will not have a charge for the services rendered. Fees are based on the federal poverty guidelines. Health Connection staff provides a good faith estimate based on the best information known and provided at the time of the estimate, it will not include any unknown or unexpected costs that may be added during your visit or treatment. The scope of services may change based on the patient's needs at the time of service, which will inadvertently affect the amount owed. You may ask for a good faith estimate by calling the clinic nearest you or in person at the clinic nearest you.

Funds are provided by Indiana Family Health Council, Inc., from the U.S. DHHS, PHS Title X Family Planning Project, and IDOH/MCH Family Planning grants in addition to patient insurance payments, fees, and donations. As public funding becomes more difficult to obtain, it is necessary for us to encourage clients to donate the maximum amount they can toward our services in order to insure the continuation of the program.
